The ÌÇÐÄÉÙÅ® Civil Rights and Title IX  Reporting Form is available to report any incident(s) involving any alleged violation(s) of the CSU Nondiscrimination Policy, which prohibits Discrimination, Harassment, Retaliation, Sexual Misconduct, Sexual Harassment, Dating Violence, Domestic Violence, Sexual Exploitation, and Stalking. You can use the form whether something happened to you or someone else, and whether it happened on or off campus.
